Course evaluation form

End-of-course evaluations work best as a short battery of ratings plus room for comments. Ratings in formset are optional unless you say otherwise, so nobody invents a score for a session they missed, and the averages come back in one request.

Say this to your assistant

“Make a course evaluation for my eight-week pottery class: rate the instruction, the materials and the pace from 1 to 5, and ask what they would change.”

Change the wording to suit your event or business. formset reads what the question is asking and picks the right type, so an email address is checked as an email and a rating is collected as a rating.

The form is hosted at its own link straight away. Respondents need no account, and you can share it as a link, a QR code, or embedded in your own site.

Then just ask

When the answers come in, ask in the same conversation:

  • “Which part of the course scored lowest?”
  • “Read me the suggestions.”
  • “Export the evaluations as CSV for the school.”
