For ChatGPT

Make forms in ChatGPT

Describe what you need to ask, and formset turns it into a hosted form with a link you can send. When people reply, ask ChatGPT what they said. The questions, the link and the answers all stay in one conversation.

1. Connect formset

formset connects to ChatGPT as a custom MCP server. You need chatgpt.com in a browser and a paid plan: Plus, Pro, Business, Enterprise or Edu. It takes about a minute, and you only do it once.

  1. Open Plugins, select Add, then Create custom MCP server. If that option is missing, turn on Developer mode in ChatGPT’s Settings first. On Business and Enterprise accounts, a workspace admin may need to allow custom servers.
  2. Name it formset and paste this address under Connection: https://mcp.getformset.com/mcp
  3. Leave Authentication on OAuth, tick I understand and want to continue, and select Create as a plugin.
  4. On the Connect formset screen, select Continue to formset, sign in with Google, and select Allow.
  5. In a new chat, select +, then formset.

The home page has the same steps with screenshots, and the documentation covers other assistants. Signing in asks Google only for your email address, name and profile picture, so formset cannot reach your Gmail, Drive, Calendar or Contacts.

2. What to say

Say who the form is for and what you need to know. For example:

  • “Make a signup for Thursday’s workshop with name, email and which session they want, morning or afternoon.”
  • “Make a quick poll for the team: which of these three dates works best for the summer party?”
  • “Make a contact form for my website and give me the embed code.”
  • “Make a pre-order form for the bake sale. Stop taking orders on Thursday at 6pm.”

ChatGPT writes the questions and picks a type for each, and formset checks every answer against its type, so an email question only accepts an email address. You get back a public link at getformset.com/f/… that works straight away. People answer in any browser with no account. Ask for a QR code for a poster, or an embed snippet for your own site that resizes itself to fit the page.

The templates have more prompts like these, with the form each one produces.

3. Can ChatGPT make a Google Form?

Not by itself. ChatGPT will happily write the questions, but it does not create the form in your Google account. People usually get from one to the other in one of two ways:

  • Copy and paste. Ask ChatGPT for the questions, then build the form in Google Forms by hand, one question and one answer type at a time.
  • Apps Script. Ask ChatGPT for a script that creates the form, then paste it into Google Apps Script, authorise it, and run it.

Either way, the answers then live in Google Forms or a linked spreadsheet. To ask ChatGPT about them, you have to export them and bring them back into the chat.

formset skips both steps. The form is built and hosted the moment you describe it, and the answers come back to the same conversation when you ask. If your organisation needs the form to live in Google Workspace, the workarounds above are still the way to do that. If you just need answers from people, a hosted link is quicker.

4. Reading the answers

Come back to the chat and ask:

  • “How did the signups split between the two sessions?”
  • “Which date is winning?”
  • “Show me everyone who rated us 2 or below.”
  • “Send me all the responses as a spreadsheet.”

formset works out the counts, averages and breakdowns, and ChatGPT turns them into an answer. An export gives you a CSV download link that works for an hour, and a read-only results page lets someone outside the chat see the latest answers for 30 days. formset makes no AI model calls of its own: answers reach ChatGPT only when you ask about them in the chat.

5. Plans

  • Free: unlimited forms, unlimited answers within fair use, answers kept for 12 months, and CSV export. Forms carry a small formset mark.
  • Pro, $12 a month: no formset mark, your own colours, password-protected forms, payment buttons on the thank-you page, answers kept as long as you want, and JSON export.

An account less than a day old can create two forms, each taking up to 25 answers, and the limit lifts by itself. See pricing and the documentation for the rest.
