Bug report form

Good bug reports have a shape: what happened, what was expected, how to reproduce it, and how bad it is. A form gives every report that shape, and asking for the severity breakdown is a faster triage than an inbox.

Say this to your assistant

“Make a bug report form: what went wrong, steps to reproduce, how severe it is, the page it happened on, and an email for follow-up.”

Change the wording to suit your event or business. formset reads what the question is asking and picks the right type, so an email address is checked as an email and a rating is collected as a rating.

The form is hosted at its own link straight away. Respondents need no account, and you can share it as a link, a QR code, or embedded in your own site.

Then just ask

When the answers come in, ask in the same conversation:

“How many blockers came in this week?”
“Show me the blocker reports with their steps.”
“Find reports that mention the checkout page.”
